Mission Healthcare provides nurse-led care at home across Dartford and north Kent. Our service is led by a UK-qualified doctor and a registered nurse, who oversee assessments and care plans and support our carers day to day. Support can range from short visits to live-in care and 24/7 care, and can be combined with personal care, specialist care or end of life care.
We work alongside the NHS professionals already involved in someone’s care, including GPs, district and community nurses, hospital discharge teams and therapists. Hands-on clinical tasks such as injections, wound dressings and catheter changes are usually carried out by community nursing teams, and our role is to provide consistent daily support, monitor wellbeing and make sure concerns reach the right people quickly.

What is skilled nursing care at home?
In England, hands-on nursing tasks in someone’s home are usually provided by NHS district or community nurses, arranged through a GP, while day-to-day care is provided by a home care service. Skilled nursing care at home describes this combination: regular, well-trained carers supporting someone with a health condition, with clinical oversight of the care plan and close communication with the nursing team. Providers of personal care must be registered with the Care Quality Commission, and you can read any provider’s inspection reports on the CQC website.
Clinical oversight of every care plan
Having a doctor and a registered nurse leading our service means care plans are built with health needs in mind, not just daily tasks. We assess risks such as falls, pressure damage, poor nutrition and medication errors, agree how our carers should record and report changes, and set out clearly when to contact a GP, the district nursing team or emergency services. Care plans are reviewed regularly and after any change in health.

Support can be short-term while someone gets back on their feet, or continue as longer-term home care if it is needed.
Funding nursing care at home
If someone’s needs are mainly health-related, they may be eligible for NHS continuing healthcare, which is fully funded by the NHS and can be fast-tracked when someone is nearing the end of life. Otherwise, care can be paid for privately or with support from Kent County Council following a free care needs assessment, often through direct payments. Benefits such as Attendance Allowance or PIP may also help.
